Visa Requirements for Application of D Visa – Malta Only
1. Completed Visa Application Form
2. One recent passport sized colour photos with white background in electronic format
3. Copy of the Valid Travel Document, the period of validity of which should exceed that of
the visa by three months.
4. Confirmed Flight Booking (stamped and signed by travel agent) or copy of actual
flight ticket showing full itinerary of your trip.
5. Confirmed Hotel Reservation
6. Health Insurance acceptable in the Schengen Area (minimum coverage has to
be 30,000 Euros) and which should cover the period of validity of visa.
7. For Minors (Under the Age of 18): Birth Certification of Minor, Authorization by both
Parents / Legal Guardians, Copy of Passport Data page of both Parent / legal
guardian , In Case of guardianship, documentary evidence proving the status.
