Fill the form,
Book an appointment
Print the application forms

I prepared the following docs.

Passport and Resiedent permit and Photocopies
Employment Letter
Payslips (3 mos updated )
Bank statements Debit and Credit cards
Travel Insurance (Accredited Schengen countries)
Confirmed Accomodation (booking.com so that you can cancel anytime)
Air ticket ( Reservartion Confirmed) go to Direct air company so that you can request a refund afterwards.
Travel plan (This one is given by the visa centre)
