Important Steps:
1) Notarise Application form , parental Authorisation Form and affidavit in lieu of original document.
2) Include Payment Receipt or money order or cashier check
3) Self attest all the document
4) self envelop or Money order for return Envelop (if not paying online)

if paying online no need to send return labels .
