Here’s what you will need:
– Application Form DS-11;
– Evidence of U.S. Citizenship;
– Proof of Identity;
– Photocopy of ID;
– Passport photo;

Just follow these steps:

Step 1: Report the Lost Passport:
Call 1-877-487-2778 to report the lost passport.

Step 2: Complete Application Form DS-11:
Do not sign the application until instructed to do so by the Acceptance Agent.

Step 3: Gather Supporting Documents:
Provide evidence of U.S. citizenship such as a certified birth certificate or previously-issued, undamaged passport;
Present a valid photo ID such as driver’s license or Naturalization Certificate;
Submit a standard passport photo taken within the last 6 months.

Step 4: Visit Application Facility:
Application acceptance facilities are located in many post offices and clerks of court;
The acceptance agent will verify your documents, administer an oath and witness the signing of the application;
The execution fee is paid to the acceptance facility;
Make payment for the application fee to the “Department of State”.
