STEP 1: Email Rome and raise your concern then wait for reply.
STEP 2: Prepare all the necessary documents such as;

Photocopy of expired passport
Photocopy of residence card
2 pc passport size photograph
Completed Form A from (Malta Passport Office Malta Website)
Confirmed Flights details

STEP 3: Go to consulate office for signing of Form A

STEP 4: Go to passport Office/Identity Malta and present all the documents.
Pay €9 for the release of document of Identity.
